Autumn Fields is a 20 bed Assisted Living Community offering living, recreational and kitchen/dining space. The home is located in Luxemburg, Wisconsin with access to community activities and services. Autumn Fields will be home to a maximum of twenty adults who have mental or physical handicapping conditions, AA-Advanced age, ALZ-irreversible dementia/Alzheimers, TBI, TI- terminally ill. Residents who have had traumatic head injuries may also be served as well as hospice services as needed. Both men and women may choose to live at Autumn Fields. The home will provide rivate rooms with handicap assessable private bathrooms for all residents. The focus of Autumn Fields in the future will be for eadults suffering with traumatic head injuries or memory impairments (Alzheimer/Dementia).

1) 24 Hour A Day Assistance With A Registered Nurse And Personnel Trained In Dementia Care

We have an RN on staff. Also a 24 hour nursing consultation available.

2) Specialized Active And Passive Activity Programs Including Spiritual Programs.

As part of our activity program we encourage the residents to do day excursions outside the facility with us.

3) Alarmed Doors With Security System And Emergency Call System.

Our facility is a secure building. All doors are locked from outside but open from inside. Due to fire codes we can not lock the doors from inside. Our emergency call system has the ability to call 911 automatically when there is a fire.

4) All Meals, Snacks And Special Diets Are Homemade And Included In The Cost Along With Housekeeping And Laundry.

5) Other Amenities

Our facility has a private movie theater, sun room, spa/tub room, manicure/pedicure services, on site playgrouond for children, raised gardens and flower bed, activity room, private dining room and 40" TV with Dishnetwork included in all the resident rooms. We encourage residents to take thier own bed as they feel more comfortable other wise we provide for them. We also accept VA residents, hospice and respite care.
